Avident Health and Unite for HER Awarded Quality Improvement Grant from Pfizer and the National Comprehensive Cancer Network® (NCCN®) to Support Metastatic Breast Cancer Patients

The collaborative project will deploy Avident Health's proprietary MyNavigator™ digital platform to enhance care delivery for patients diagnosed with metastatic breast cancer (mBC). The initiative aims to bridge critical communication gaps between clinic visits by delivering personalized, patient-friendly education aligned with NCCN Guidelines for Patients®, driving active enrollment and utilization of Unite for HER's free supportive services, and equipping informal family caregivers to foster shared decision-making, treatment adherence, and enhanced quality of life.

Living with metastatic breast cancer requires complex, ongoing treatment management where adherence, symptom monitoring, and whole-person supportive care are paramount. However, patients and their loved ones often struggle to navigate complex clinical guidelines and access community-based supportive resources outside traditional clinical appointments.

Through this grant-funded initiative, the MyNavigator platform will serve as an interactive, home-based companion that:

• Delivers Tailored NCCN Guideline Education: Translates complex clinical pathways into accessible, bite-sized modules tailored to each patient's specific treatment journey, empowering patients to participate actively in informed clinical decision-making.

• Streamlines Access to Integrative Supportive Care: Directly integrates Unite for HER's comprehensive suite of free services—including professional nutrition counseling, fresh food delivery, mental health counseling, and evidence-based integrative therapies—directly into daily navigation prompts.

• Engages the Informal Caregiver: Uniquely includes family members and daily care partners in a secure, proactive communication loop, ensuring they have the context, education, and early symptom alerting needed to support treatment adherence at home.

• Enables Proactive Care Team Interventions: Captures patient-reported check-ins and symptom trends to alert clinical teams early, preventing avoidable complications and reducing acute care utilization.
