Smart Alerts Boost Bystander CPR Response Before Ambulance Arrival

TLDR

  • Smart alerting in Freiburg's ALARM trial matched large-circle volunteer arrival rates while cutting wasted dispatches by 31%, giving systems an edge in efficiency.
  • The ALARM trial randomly assigned three alerting strategies daily, using GPS-measured arrival times to compare smart travel-time calculations against fixed geographic radii.
  • Smart alerting helps more cardiac arrest patients receive faster bystander CPR, potentially saving lives and making communities healthier and more resilient.
  • Smart alerting nearly doubled volunteer arrival before EMS compared to small circles, showing that travel time beats straight-line distance in emergency response.

Impact - Why it Matters

This news matters because out-of-hospital cardiac arrest is a time-critical emergency where every minute without CPR reduces survival chances. Current alerting systems often rely on fixed geographic circles, which this study shows can miss faster responders and send volunteers who arrive too late. By using smart alerting that calculates actual travel time, dispatch centers can more efficiently mobilize trained volunteers, potentially getting CPR and AEDs to patients faster. With more than half of patients receiving volunteer care before EMS in the smart alerting group, this approach could translate into more lives saved. As communities worldwide look to improve cardiac arrest response, these findings offer a practical, data-driven way to optimize existing volunteer networks without adding resources.

Summary

New late-breaking research from Freiburg, Germany, suggests that being “smart” about which volunteer community first responders are alerted to a cardiac arrest can significantly improve the chances of a trained volunteer reaching the patient before emergency medical services (EMS) arrive. The findings, presented at the American Heart Association’s Resuscitation Science Symposium 2026 in Chicago, come from the ALARM randomized clinical trial, which compared three alerting strategies for smartphone-based first responder systems. The study was conducted by the nonprofit Region of Lifesavers (Region der Lebensretter e.V.), which operates the alerting system in the Freiburg region.

During the seven-month trial, the system randomly switched daily among three strategies: alerting volunteers within a small geographic circle of about 600 meters, alerting those within a larger circle of about 2,500 meters, or using a “smart alerting” calculation that considered expected travel time to the patient relative to ambulance arrival. Results showed that a trained volunteer arrived before EMS in only 31% of emergencies with the small circle, but in about 57% with the large circle and 58% with smart alerting. Smart alerting was also more efficient: it deliberately did not send 31% of volunteers who accepted an alert because they would likely arrive after EMS, reducing unnecessary dispatches. “When the app evaluated who can actually get there fastest, instead of just who is closest on a map, more than half of patients had a trained volunteer providing care before the ambulance arrived,” said Michael Mueller, M.D., senior study author and chair of anesthesiology, intensive care and emergency medicine at St. Josef’s Hospital in Freiburg. The researchers noted that further studies are needed to determine whether smart alerting improves cardiac arrest survival.

The American Heart Association’s 2025 Emergency Cardiovascular Care Guidelines emphasize that prompt CPR and defibrillation greatly increase survival chances. While the study did not measure patient outcomes, Mueller said, “Whether smart alerting improves survival odds needs to be examined in future research.” The trial was simultaneously published as a full manuscript in the Journal of the American Heart Association.
